First Minnesota Care is a Personal Care Provider Organization established to provide personal care services to individuals in their homes. A registered nurse also provides nursing supervision.

Services are available to individuals with disabilities or chronic health conditions and to elderly persons who require assistance to remain independent in their homes.

Personal Care Program
We provide all services available in the Personal Care Program, and those services may be used to provide assistance at home, school, or other places in the community where help is required. To be eligible for the program, you must be eligible for Medicaid and under a physician’s care. Prior authorization is required for all services.

PCA Choice Services
We also provide PCA Choice Services. This program allows you to manage your care plan and to recruit, hire, orient, and supervise your caregivers. You and members of your family can decide who provides services, and when you want to use those services. We act as a fiscal agent to manage billing and payroll activities.

Fully Authorized
First Minnesota Care has been approved by the Minnesota Department of Human Services to provide Personal Care Assistant Services. as well as services under the PCA Choice Program, to residents of the Minneapolis, St. Paul metropolitan area.

Interpreter and Translation Services
We also offer interpreter and translation services for members of the Somali community.
